About The Position

Sentara RMH Medical Center is now hiring an Inpatient Case Manager, Social Worker. This is a full-time, day shift position that will be responsible for the provision of medical social work services to patients receiving care in the hospital setting.

Requirements

• Master's Degree in Social Work.
• 1 year of Case Management or other related experience.
• Accredited Case Manager RN (ACM-RN) certification within 1 year of hire.
• Basic Life Support (BLS) certification within 90 days of hire.
• Certified Case Manager (CCM) certification within 1 year of hire.

Responsibilities

• Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
• Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs for each referred patient.
• Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.
• Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals.
