The Inpatient Care Manager position at Advocate Condell Medical Center, part of Advocate Health, is a PRN (part-time, as needed) role with Monday to Friday, 8-hour shifts from 8am-4:30pm. This role is responsible for conducting comprehensive patient assessments, establishing appropriate care plans, and initiating timely interventions. Key aspects include collaborating with patients, families, and healthcare teams to achieve patient and organizational goals, particularly in discharge planning and care transitions. The manager will utilize patient/family strengths in problem-solving, provide continuity of care compliant with regulatory standards, and manage various healthcare-related issues such as finances, housing, and family dynamics. Responsibilities also involve initiating internal and external referrals, documenting discharge planning and utilization review activities, and performing concurrent and retrospective reviews. The Inpatient Care Manager works closely with Social Work and support personnel, serves as an active member of the Outcome Facilitation Team, and collaborates with medical staff and other departments to resolve barriers to discharge and expedite care. They will also provide clinical updates to payers, manage denial activity, and deliver CMS regulatory notices. The role requires developing and maintaining relationships with community-based agencies and serving as an educator and expert resource on admission status, acute care criteria, utilization management, care coordination, and regulatory requirements.
