Join our clinically strong and cohesive-always collaborative and supportive- Inpatient Care Management team as an Inpatient Care Manager, where you'll grow and thrive in a setting that encourages bold thinking, curious learning and co-creation. Your role will involve the initial and concurrent utilization review, discharge planning, coordination of care and referrals for hospitalized members. You will collaborate with physicians, the care team and our Medical Directors to ensure the patient is at the right place, the right level of care, and at the right time-and you will assess, coordinate and implement care plans to ensure the patient’s needs are met across the continuum of care - targeting optimal experience, outcomes and cost. As an Inpatient Care Manager, you'll establish relationships and foster respect and rapport with our Health Management Operations and Health Value Teams and will work closely with our Provider Partners from the health care team responsible for the management and coordination for our hospitalized members at risk, and at their most vulnerable. We seek individuals with a strong commitment to patient-centered care and a passion for promoting health, wellness, and patient as well as provider experience. Your focus will be on: Conducting Initial and Concurrent Review and Discharge Planning/Care Coordination, utilizing evidence-based criteria coupled with your clinical expertise and collaboration with our Medical Directors to ensure appropriate admissions, level of care and timely discharges. Monitoring and continually assessing and evaluating member severity of illness, intensity of services and response to treatment, making recommendations for appropriate continued stay, transition to lower level/tertiary care and coordinate member transitions. Applying clinical knowledge, critical thinking skills, as well as your solutions-oriented problem-solving and strong decision-making approaches.

Requirements

  • Current unrestricted California RN licensure require.
  • Minimum of 3-5 years recent acute/hospital or related clinical experience
  • Working knowledge and application of utilization management standards, medical necessity criteria which involves the organization, extraction and assimilation of relevant and appropriate clinical data from medical records, and utilization of evidenced-based criteria based on nationally recognized treatment standards.
  • Employment of effective and meaningful communication skills-both verbal and written- with both our internal and external customers, aimed at advocacy for patient experience, optimal outcomes and accuracy of patient record.
  • Development of collaborative relationships and work with our Providers and fostering a shared vision and values.
  • Computer skills proficient, experience in EMR, Microsoft Office Suite.

Nice To Haves

  • Bachelor of Science- Nursing preferred.
  • Desired experience in managed care concepts and practice.
