It’s the way you want to be treated. The position involves monitoring, coordinating, and evaluating the care and appropriateness of hospitalization, as well as initiating referrals. The role also includes participation in the development, implementation, and analysis of care delivery standards and outcome measures.

Requirements

  • Currently licensed as a Registered Nurse in the state of Illinois.
  • BSN preferred.
  • 2-5 years previous Case Management experience preferred.
  • Current CPR certification.
  • Relevant hospital nursing; hospital case management; insurance case management experience preferred.

Responsibilities

  • Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families, and multidisciplinary care team respecting choice of patient.
  • Concurrently reviews clinical documentation in medical record to assure timely progression of care transitions.
  • Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely and appropriate progression of care and eliminates barriers for next site of care transitions.
  • Assists in expediting care delivery to avoid delays in timely service provision.
  • Provides feedback to medical staff and conducts education as expert resource regarding care coordination, discharge planning needs, and regulatory requirements.
  • Delivers CMS regulatory notices within CMS established timeframes.
  • Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits.
  • Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct.
