A Registered Nurse for Home Infusion inserts, monitors and removes an IV used to give medications, blood products and nutrition to a patient. During the IV therapy, the nurse monitors the patient for adverse reactions and ensures that the tubing, bandages and needle stay in place. When a patient needs blood levels checked, an IV nurse performs veinous phlebotomy. During the entire IV therapy process, the nurse obtains sets of vital signs, completes the physical assessment and review of systems, documents all patient-related activities and records the medications she uses. Provides and coordinates all aspects of staffing services to the patient with the assistance of the Company Directors, Nursing Coordinator and the Director of Nursing. Communicates with the patients to determine problems and/or solutions regarding their treatment. The Per diem RN also conveys collected documentation from patient to all necessary internal and third-party external staff regarding scheduling and other pertinent patient information. Performs a variety of duties both clinical and nursing notes documentation. This position requires skill in planning, organizing, scheduling and coordinating the delivery of patient care by all staff personnel including but not limited to complete documentation of nursing care and submission of nursing notes.

Requirements

  • Nursing Degree from an accredited college
  • IV certification & experience in central line care and management
  • Current state RN license
  • Minimum of two years nursing experience
  • Valid state driver’s license
  • Experience with medical terminology
  • Experience in home care, critical care, emergency nursing, AIC, or hospital discharge planning
  • Some experience in staffing
  • Ability to multi-task
  • Good communicative and interpersonal skills
  • Basic level skill in Microsoft Office (including Word, Excel, PowerPoint)
  • Willing to travel to see patients in the homes and in other health care settings
  • Experience providing customer service to internal and external customers, including meeting quality standards for services and evaluation of customer satisfaction
  • Excellent verbal skills
  • Good written skills
  • Possess medical-surgical experience or knowledge in phlebotomy and venous access
  • Ability to work independently, as well as on a team
  • Ability to work under high levels of stress and handle critical situations in the field
  • Excellent Customer Service skills
  • Proficiency in all programs necessary for execution of position
  • CPR certification

Responsibilities

  • Perform a variety of duties both clinical and nursing notes documentation.
  • Plan, organize, schedule, and coordinate the delivery of patient care by all staff personnel.
  • Complete documentation of nursing care and submission of nursing notes.
  • Utilize the nursing process to schedule nursing visits in accordance with RX’s and established protocols.
  • Coordinate SOC and ongoing nursing visits and confirm scheduled visits with both patient and Nursing Agency/RN.
  • Assure schedule is sent out via email to the team.
  • Follow up on and enter Therapy Admin reports and email to the team.
  • Review all nurses notes for content, accuracy, and completion without omitting necessary information such as drugs given, drug dosing, and time of administration.
  • Maintain continual communication with families, supervisors, and field personnel/agencies regarding problems and/or special needs of the patient.
  • Communicate all problems and reportable situations to the referring MD, appropriate MD office administrator, and LUX Infusion nursing.
  • Perform computer functions including input, reports, and searches.
  • Communicate patient/employee information to answering services when necessary.
  • Read and understand company compliance paperwork and policies.
  • Perform all duties in accordance with all applicable federal and state laws and with the highest ethical standards.
  • Coordinate staffing in a timely manner while adhering to confidentiality and ethics.
  • Administer prescribed intravenous solutions and medications; monitor & maintain infusion sites & system; evaluate responses to prescribed therapy.
  • Teach patients & caregivers to perform & monitor administration of Home Infusion as needed.
  • Prepare clinical records, care plans, and progress notes for each patient.
  • Coordinate schedule of patient visits & phone calls.
  • Work in patients' places of residence.
  • Drive as assigned within territory.
  • Prioritize and handle multiple tasks and projects.
  • Work extra hours if needed due to scheduling flexibility.
  • Pay careful attention to detail.
  • Perform related duties as assigned.
  • Participate in quality assurance activities and audits as directed.
  • Provide professional nursing care within the department.
  • Coordinate care planning with other disciplines.
  • Provide care that meets the psychosocial, physical, and general aspects of care.
  • Determine priorities of care based on physical and psychosocial needs of patients, as well as factors influencing patient flow through the system.
  • Communicate regarding changes in patient status, symptomatology, and results of diagnostic studies.
  • Respond quickly and accurately to changes in condition or response to treatment.
  • Develop caring relationships with patients and treat them with respect.
  • Teach the patient how to properly care for their IV or PICC line and communicate with patients clearly while giving instructions.
