Job Description Summary: Schedule: 2-3 shifts a week Mon-Fri, no weekends & no holidays · Hours of Operation: 9:00am - 5:30pm Come join the OhioHealth Infusion Pharmacy team! This role offers a dynamic environment where you can collaborate with team members to provide care to patients transitioning from the hospital to their homes. You will be an integral part of the referral process to ensure our patients are set up for success by utilizing your clinical expertise, great communication skills and attention to detail. In addition, you will have the opportunity to provide patient education on how to setup their IV hookups independently. You will be joining a collaborative team of Pharmacists, Nurses, and Technicians. Orientation is typically 12 weeks and we have a great learning culture with tenured staff of 20+ years to support onboarding needs. The Intake RN is responsible for the screening of incoming calls, obtaining any relevant nursing and medical information necessary to interpret and facilitate the needs of the caller, referral coordination, ensuring physician orders for services are in place, physician maintenance, ICD-10 coding, processing referral information and for guiding non-professional staff through referral issues for Home Health and IV Pharmacy referrals. In addition to providing virtual and in person IV education to patients at bedside.

Requirements

  • Associate's Degree (Required)
  • RN - Registered Nurse - Ohio Board of Nursing

Nice To Haves

  • 2-3 years of bedside/floor nursing experience in IV Therapy
  • Transitions of Care experience is a plus!

Responsibilities

  • Receives new referrals and researches clinical information required for service.
  • Confirms that the patient is in the service area meets initial requirements for referred services.
  • Assists Intake Specialists with referral questions, medical terminology and interpretation of physician orders.
  • Case coordination for Home Health and/or Pharmacy service s .
  • Identification of diagnosis for care and assignment of ICD-10 codes.
  • Referral data entry.
  • Assesses referral information to ensure orders for service are complete and appropriate to the patient’s diagnosis.
  • Completes physician orders as needed.
  • Knowledgeable of Medicare, Medicaid and insurance guidelines.
  • Verifies payer eligibility when the absence of the Intake Specialist .
  • Refers any questionable referral to the Intake Supervisor.
  • Coordinates communication with all required departments to assure pending referrals are coordinated in a timely manner.
  • Acts as an agency referral source for referring clients to other community services/agencies.
  • Knowledgeable about services, resources and physicians in the community.
  • Participates in new Intake staff orientation.
  • Engagement and taking an active role in development and the betterment of the associate and the organization
  • Completion of duties as assigned that pertain to compliance, quality, culture, and excellence of the organization
