Under the direct supervision of the Coordinated Services Program Supervisor, the Case Manager Lead is responsible for assisting people living with HIV access the continuum of HIV care services. Engage, screen, and refer clients living with mental health, substance abuse or dual disorders in HIV care and treatment services. The Case Manager Lead will be responsible for providing direct services, and coordinating services for clients who are HIV+ and conducting a comprehensive uniform assessment in the following areas: health status, medical care and provider, activities of daily living and mental health status. The Case Manager Lead will be responsible for maintaining comprehensive files for each client and developing a care plan based on client needs. Will represent the HIV department in the community, interacting and networking with community service agencies that provide services to people living with HIV and their families. Cultivates and maintains communication and networking with multi-ethnic organizations in the HIV community. The Case Management Lead will serve as a role model for cultural and linguistic competency. Maintain client confidentiality at all levels.

  • Develop excellence in case management services which will enrich and enhance the quality of life, dignity and respect of people living with HIV
  • Oversee day-to-day operations of Case Management program, providing guidance, support and resources when needed
  • Review client files, ensuring all required data is included and entered in ARIES; ensure comprehensive assessments and case plans have accurate documentation of referrals and follow-up
  • Responsible for facilitation of interdisciplinary case conferencing meetings, preparation of agenda, meeting notes and action plan
  • Conduct quality improvement and evaluation activities through weekly Case Conferencing meetings, weekly new case reviews, and other activities as directed by supervisor or manager
  • Timely professional maintenance of records and documentation, including progress notes, treatment plans and ongoing evaluation of client progress, consistent with agency expectations and ethical standards
  • Conduct a comprehensive assessment and administer an acuity scale to determine the medical, psychosocial, and social service needs of clients
  • Ensure clients are enrolled in medical care and receive appropriate referrals to needed services as established in case plan
  • Advocate for clients and assist them in the procurement of benefits and services
  • Maintain a caseload of 30-50 clients
  • Three (3) years of experience working in human or social services
  • Experience with and sensitivity to LGBT-related cultural concerns
  • Prior case management or counseling experience and experience in group facilitation and educational instruction
  • Experience with Motivational Interviewing and the Harm Reduction Movement
  • Bachelor’s or Associate’s degree in social work or related field; or a registered nurse, or a certified substance abuse counselor and a minimum of three to four years professional or volunteer experience in the field of HIV or related social services field, and familiarity with HIV services; or High School diploma and a minimum of three years of experience in HIV Case Management
  • Bilingual (English/Spanish) required
  • Knowledge of health and psychosocial issues concomitant to HIV disease
  • Computer literacy required, proficiency in MS Office (Word, Excel, Outlook and PowerPoint), projectors, various software including ARIES
