Infectious Disease RN Care Coordinator
About The Position
As an RN Care Coordinator for Infectious Disease at Virginia Mason Franciscan Health, you will play a pivotal role in patient care, guiding individuals through their healthcare journey to achieve optimal outcomes and an exceptional patient experience. You will collaborate with a team of leading experts in Infectious Diseases, providing individualized care for patients aged 16 and older with a wide range of infectious diseases. Your daily responsibilities will involve strategically assessing, planning, and facilitating comprehensive care across the entire continuum. You will expertly advocate for patients while working closely with physicians, nursing staff, various departments, insurers, and post-acute providers to ensure smooth, high-quality transitions in care. Success in this role requires strong clinical acumen, excellent communication and advocacy skills, and a strategic mindset, all driven by a commitment to optimizing patient care at every stage.
Requirements
- Graduate of an accredited school of Nursing
- Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of one (1) year experience
- Current licensure as a Registered Nurse in the state of Washington (RN-WA)
- BLS required within three (3) months of hiring if located within hospital
- Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used
- Able to apply clinical guidelines to ensure progression of care
- Must have critical thinking and problem-solving skills
- Collaborate effectively with multiple stakeholders
- Professional communication skills
- Understand how utilization management and case management programs integrate
- Ability to work as a team player and assist other members of the team where needed
- Thrive in a fast paced, self-directed environment
- Knowledge of CMS standards and requirements
- Proficient in prioritizing work and delegating where indicated
- Highly organized with excellent time management skills
Nice To Haves
- Experience managing patients with chronic disease in an ambulatory setting
- Bachelor's Degree in Nursing (BSN) or related healthcare field
- At least five (5) years of nursing experience
- Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification
- Knowledge of managed care and payer environment
