Make a difference As part of a very progressive Ambulatory Care pharmacy team at Community Health Network, you will participate in growing pharmacy services within Infectious Disease Clinics using a collaborative, team based approach. By means of a collaborative drug therapy agreement, you may make medication changes, as appropriate. You will do everything from collaborate with Infectious Diseases (ID) physicians for optimal HIV, PreP and antifungal medication selection and monitoring, participate in managing patients receiving outpatient parenteral antimicrobial therapy (OPAT), aid in ambulatory antimicrobial stewardship efforts, and serve as a resource for ID-related drug information questions. Other aspects of this position include taking a leadership role with staff education, guideline/policy development and implementation, and participation in performance improvement initiatives. You will have opportunities to mentor and teach pharmacy students and pharmacy residents along with serving as a clinical resource for the pharmacy staff. Your expertise in your field will allow you to advance the level of patient care to achieve the best possible outcomes.
