The Infection Preventionist (IP) works directly with all employees, patients, medical providers, and pharmacists to fully implement all aspects of the Center’s Infection Prevention and Control Program (IPCP); which is designed to comply with all aspects of CDC, CMS, OSHA, and other federal and state guidelines for long-term care.

  • Plans, implements, directs, and coordinates the Center’s Infection Prevention and Control Program.
  • Provides IPCP training for employees, patients, visitors, and contracted persons at the time of orientation, admission, or initial contact at least annually and as needed.
  • Conducts compliance audits within the Center to validate the Center’s adherence to the IPCP.
  • Works collaboratively with Center employees to promote employee contributions to the IPCP planning at the center and corporate levels.
  • Works closely with the Clinical Education Director in the development of policies, procedures, protocols, and training programs to improve infection prevention and control activities that promote patient care and safety.
  • Performs daily, weekly and monthly rounds. Records findings of rounds and maintains a record from one survey period to the next. Develops and implements an action plan to resolve infection prevention and control concerns identified on rounds.
  • Using the CCR IPCP Meeting Agenda the IP is responsible and accountable for having a monthly IPCP/Antibiotic Stewardship Program committee meeting. Maintains a list of attendees, ensures minutes are recorded and reflect a summary of committee discussions relative to patterns and trends of infections in the center.
  • Plans, implements, directs, and coordinates the center’s Antibiotic Stewardship Program. Works collaboratively with the IDT, Pharmacist, Medical Director, Prescribers, and nursing employees to appropriately implement the antibiotic stewardship program.
  • Prepares the monthly infection control and antibiotic stewardship reports and submits the reports to the Director of Nursing, no later than the 10th day of the following month.
  • Collaborates with the QAPI team to identify and resolve identified areas of infection prevention and control concerns.
  • Meets with the IDT to determine type and duration of transmission-based precaution and co-horting needs as indicated.
  • Ensures appropriate transmission-based isolation signage is posted and based on the organism’s route of transmission.
  • Ensures transmission-based precaution sign(s) are located and easily seen outside of the patient’s room.
  • Ensures transmission-based precaution isolation supplies are readily available and near the entrance to the patient’s room.
  • Ensures transmission-based precautions regulated medical waste supplies are available inside the patient’s room.
  • Provides in-person infection prevention and control education to employees at the time of hire, annually and as needed.
  • Coordinates and manages vaccine administration (influenza, hepatitis B, COVID-19, etc.), as indicated for employees and patients.
  • Maintains appropriate infection surveillance and illness logs for employees and patients as indicated.
  • Responsible for performing tuberculin skin test (TST) for employees. Responsible for tracking and keeping records of TST administration and results for patients and employees.
  • Conducts, attends, and participates in orientation, training and in-service education and staff meetings.
  • Enforces the center’s safety and health programs and addresses all physical hazards.
  • Ensures that all alleged violations involving mistreatment, neglect, or abuse, including any injuries of unknown source, are reported immediately to the Administrator. Investigates and takes action to prevent further problems. Follows CCR policy.
  • Actively participates in center committees as assigned.
  • Maintains current knowledge and understanding of policies, procedures, and organization of the center.
  • Attends corporate level education programs and meetings as indicated.
  • Perform other duties as assigned.
  • Graduate of an accredited school of nursing, currently has a LPN license, and in good standing with the Virginia Department of Health Professions.
  • Demonstrates completion of specialized training in infection prevention and control prior to assuming responsibility for the center’s infection prevention and control program. Proof of specialized training is required.
  • Exceptional clinical and customer service skills.
  • Ability to work independently, self-motivate, and effectively communicate with others.
  • Knowledge of federal, state, and licensure regulations related to long-term care.
  • Proficient in verbal and written communication skills.
  • Ability to work independently, prioritize responsibilities and adjust to rapidly changing priorities.
  • Proficiency in Excel.
  • Must be able to lift 25 pounds at a time
  • Must be able to move 150 pounds with assistance
