About The Position

Vasona Creek Healthcare is seeking an experienced and motivated Infection Prevention (IP) Nurse – LVN to join our team. This full-time, benefitted position is responsible for leading and maintaining an effective infection prevention and control program to ensure a safe environment for residents, staff, and visitors. Requirements

  • Active LVN license in good standing
  • Infection Prevention experience in skilled nursing or healthcare preferred
  • Knowledge of CDC guidelines, NHSN reporting, and regulatory requirements
  • Strong organizational, communication, and analytical skills
  • Ability to work independently and collaboratively

Responsibilities

  • Oversee and manage the facility’s Infection Prevention & Control Program
  • Monitor, investigate, track, and report infections per CDC, state, and federal guidelines
  • Educate and train staff on infection prevention policies and best practices
  • Conduct audits, surveillance, and risk assessments; analyze trends and outcomes
  • Collaborate with nursing leadership and interdisciplinary teams
  • Prepare and maintain documentation for regulatory compliance and surveys
  • Ensure adherence to OSHA, CDC, and public health standards
