About The Position

Cooperative for Human Services, Inc. is seeking an Individual Supports Program Coordinator to support individuals with developmental disabilities across the Lowell, Central Middlesex, and Merrimack Valley regions. This is a highly engaging, field-based role for someone who is organized, compassionate, and comfortable balancing direct supports, case coordination, documentation, and staff supervision. This role plays a key part in ensuring individuals receive high-quality, person-centered services while maintaining compliance with DDS and CHS standards.

Requirements

  • At least two (2) years of experience supporting individuals with developmental disabilities
  • At least one (1) year of supervisory experience
  • Strong organizational skills and comfort managing a caseload with billable hours and documentation requirements
  • Computer competency and ability to learn new systems (experience with iCentrix preferred, not required)
  • Reliable personal vehicle and valid driver’s license (required for regular community-based travel)
  • Ability to lift up to 30 pounds and meet the physical demands of a community-based role
  • Strong written and verbal communication skills
  • Ability to exercise sound judgment and work independently in dynamic environments
  • Associate’s Degree in a related field required; Bachelor’s Degree preferred

Responsibilities

  • Serve as an advocate for individuals receiving services, ensuring their rights, dignity, and voices are respected
  • Provide direct supports, case management, and case coordination services to a caseload of individuals in the community
  • With the support of your manager; develop, implement, and monitor Individual Support Plans (ISPs) in collaboration with interdisciplinary teams
  • Maintain regular communication with individuals served, families/guardians, DDS Service Coordinators, medical and behavioral health providers, and internal CHS partners
  • Track and manage billable service hours in accordance with program and funding requirements
  • Complete required documentation, case notes, reports, and data collection accurately and on time
  • Participate in a rotating on-call schedule (Thursday–Thursday) and respond to urgent situations as needed
  • Travel regularly throughout the service area using your personal vehicle (mileage reimbursed)
  • Support individuals in accessing community resources, medical and behavioral health services, and natural supports
  • Promote health, safety, and wellbeing, including oversight of medication administration, basic home safety, and healthy lifestyle supports
  • Ensure compliance with DDS regulations, CHS policies, and all applicable state and federal requirements
