About The Position

Cooperative for Human Services Inc. is a multi-service organization that provides residential services, independent living supports, and guardianship services for adults with intellectual, developmental, and other related disabilities. We are seeking a compassionate Individual Supports Program Coordinator to join our team and deliver exceptional care and support to the individuals we serve. We have full-time and part-time hours available in Malden, MA, working Sunday - Thursday, 11 AM - 7 PM

Requirements

  • High School diploma or equivalent required
  • Minimum 1 year of previous work experience with adults with disabilities required
  • First Aid and CPR Certification required before starting employment
  • Ability to lift 30lbs+
  • Ability to fluently speak, read, and write in English
  • Effective communication (verbal & written) and problem-solving skills
  • Ability to understand and comply with multiple regulatory requirements
  • A valid Massachusetts/New Hampshire/Rhode Island driver's license, a reliable vehicle, and a good driving history record

Responsibilities

  • Act as a liaison to and communicate regularly with the individual served, family members and/or guardians, other service providers, management personnel, medical personnel, psychologists and/or psychiatrists, funding and referral sources, including DDS Services Coordinator and other stakeholders of the organization, as assigned, in a professional manner.
  • Assist in obtaining and arranging for additional services such as medical, dental, psychiatrist, and any other services necessary. Attend said service appointments as needed.
  • Provide on-call assistance and respond to emergency situations.
  • Assist in the development of each person’s ISP. Assist in the development of individualized goals and objectives that reflect the preferences, needs, and desires of the persons served in conjunction with members of his or her support team.
  • Assist in the development of and provide training to persons served in order to meet identified needs.
  • Provide statistical and narrative data on a monthly, quarterly, and yearly basis to support program evaluation measures, family assessments, utilization, incidents, etc. Provide written reports as required.
  • Develop positive relationships and community connections utilizing natural support strategies.
  • Maintain informal and formal communication with the supervision and provides written communication when required.
