About The Position

The position involves providing comprehensive case management services to a challenging caseload of mentally ill, homeless adults. Responsibilities include assisting clients with obtaining benefits, referrals for treatment, monitoring hygiene, teaching independent living skills, and providing advocacy for entitlements and housing appointments. The role also includes facilitating weekly psycho-educational groups, generating ongoing progress and group notes, and preparing monthly statistical reports. Participation in clinical team meetings, case conferences, and other staff meetings is required, along with assisting with program staff coverage as needed. Emergency first aid/CPR assistance may also be required, along with performing additional related duties as assigned.

Requirements

  • High School Diploma/GED required; bachelor's degree preferred and master's ideal.
  • Bilingual Spanish strongly preferred, but not required.
  • Excellent communication and writing skills.
  • Computer proficiency required.
  • Experience working in residential treatment and with MICA and homeless populations.
  • Knowledge of HRA and NY/NY application systems a plus.
  • CPR training certification or willingness to take training class in CPR.

Responsibilities
