The position involves providing comprehensive case management services to a challenging caseload of mentally ill, homeless adults. Responsibilities include assisting clients with obtaining benefits, referrals for treatment, monitoring hygiene, teaching independent living skills, providing escorts to and advocacy for entitlements and housing appointments, and facilitating housing placement. The role also requires facilitating weekly psycho-educational groups, generating ongoing progress and group notes, and preparing monthly statistical reports. Participation in clinical team meetings, case conferences, and other staff meetings is expected, along with assisting with program staff coverage as needed. Emergency first aid/CPR assistance may be required, and additional related duties may be assigned.
