About The Position

The position involves being responsible for all aspects of an assigned caseload, which includes meeting with clients living in a Community Residence at least weekly. The role requires making referrals to medical, mental health, and/or substance abuse services as needed, while continuing to involve providers in the clients' care. Supportive counseling is provided to clients, along with regular visits to their rooms to monitor their safety and security. The position also involves teaching and assisting clients with activities of daily living, completing client 2010 e housing applications, service plans, and psychosocial reports. Appropriate levels of support, including escorts if necessary, are provided to clients. It is essential to ensure that all clinical files and reports are complete and up to date, assist with the intake and admission process, participate in community meetings, and provide emergency first aid/CPR assistance when needed. Related duties may be assigned as necessary.

Requirements

  • BA preferred
  • HS diploma/GED required
  • Significant experience working with the substance abuse and/or mentally ill population required
  • Excellent writing skills
  • Basic computer literacy required
  • CPR training certification or willingness to take training class in CPR
