Acts as the primary investigator for all incidents reported to the NYS Justice Center for assigned region and other sensitive situations as requested, collecting information from multiple sources and ensuring investigations are completed thoroughly and timely, in accordance with applicable regulations. Writes and submits complete investigative reports to be reviewed by the agency’s Incident Review Committee Monitors IRMA daily and informs programs within their region when information is requested and required. Follows up with programs as needed until all required information is entered in IRMA and the incident is ready for closure. Assists Regional Director(s) with identifying trained investigators within the region to investigate minor notable occurrences and other incidents that do not rise to the level of a reportable incident. Reviews investigations completed by other trained investigators in their assigned region prior to submission to the IRC for review. Responds to requests for information/updates regarding investigations from the OPWDD Incident Management Unit (IMU) and the NYS Justice Center. Assists with preparations for the bi-monthly IRC meetings by reviewing submitted investigative packets and follow up submitted by programs to ensure they contain all required elements and are ready for review. Assists program staff with entering required information into IRMA and closing incidents as needed. Provides training in the incident management process and responsibilities of mandated reporters to program staff as needed. Presents data on incident trends and new OPWDD regulations on incident management to program staff during leadership meetings or as requested. Participates in internal and external committees as appropriate, which may include OPWDD Mortality Review and provider organization meetings. May perform data analysis relating to investigations and trends in incident classifications and substantiated/ founded incidents. May compile regional and agencywide trend reports relating to reportable incidents. May maintain a tracking system for reportable incidents by reviewing and entering data. May develop and facilitate training for program operations staff relating to participating in the investigative process, submitting written statements, timely incident reporting, and other incident management topics as needed and annual refresher training for other trained investigators within the agency. Performs all other duties as assigned.
