About Concordia Hospice of Washington The roots of our organization can be traced back to Rev. Dr. J. E. Victor Carlson, who helped create Hospice Care Inc. in Greene County in the late 1970s to serve families dealing with end-of-life illnesses. In 1996, the program was combined with The Washington Hospital, opening opportunities for inpatient care and other new services. On July 1, 2016, Concordia Community Support Services and Washington Health System (previously The Washington Hospital) partnered to form the organization we are today. Requirements

  • Adaptability, compassion, and a commitment to patient-centered care.
  • Ability to work independently and manage time effectively.
  • A minimum of 2 years of nursing experience, preferably with experience in palliative care, hospice, or oncology.

Responsibilities

  • Assessing and managing patient needs: Continuously assess the physical, emotional, and spiritual needs of patients, developing individualized care plans with the interdisciplinary team.
  • Providing expert symptom management: Administer medications and treatments, focusing on pain management and symptom control to improve the patient's quality of life.
  • Educating and supporting families: Be a trusted resource for families, providing education about the disease process, care options, and end-of-life decisions, while offering emotional support.
