About The Position

The In-Home Primary Care Medical Director provides clinical oversight and leadership for the organization’s in-home primary care program. This role supports high-quality patient care through expert guidance, chart review, and collaboration with Nurse Practitioners, Physicians, and other clinical providers. The Medical Director ensures clinical excellence, regulatory compliance, and adherence to evidence-based standards of practice. This is a part-time position requiring approximately 6–10 hours per month.

Requirements

  • Practicing physician currently licensed under the Oregon Medical Board.
  • Board certification in a relevant specialty (e.g., Family Medicine, Internal Medicine, Geriatrics).
  • 3–5 years of clinical experience in primary care, geriatrics, home health, or related field; prior medical director or chart-review experience preferred.
  • Clinical Skills: Proficient in the assessment, diagnosis, and management of a wide range of medical conditions, with a focus on preventive care and chronic disease management.
  • Strong ability to review clinical documentation, evaluate quality of care, and provide constructive feedback.
  • Excellent communication and interpersonal skills with a strong commitment to patient-centered care and collaborative practice.
  • Technology Proficiency: Familiarity with electronic medical record (EMR) systems and telehealth technologies.
  • Commitment to patient-centered care, health equity, and high-quality home-based service delivery.

Responsibilities

  • Conduct regular clinical chart reviews for Nurse Practitioners, Physicians, and other clinicians to ensure adherence to clinical standards, regulatory requirements, and organizational policies.
  • Provide timely feedback and recommendations to support continuous improvement and professional development of clinical staff.
  • Monitor documentation quality, appropriate diagnosis and treatment planning, medication management, and follow-up care.
  • Serve as a clinical resource to Nurse Practitioners, Physicians, and other care team members, responding to questions and providing guidance as needed.
  • Offer expert consultation on complex patient cases, diagnostic challenges, treatment decisions, and care coordination concerns.
  • Support clinicians in delivering safe, effective, and patient-centered care within home-based settings.
  • Collaborate with leadership to support development of clinical protocols and best practices for in-home primary care.
  • Participate in quality assurance activities, clinical audits, and continuous quality improvement initiatives.
  • Maintain familiarity with current medical guidelines, regulatory updates, and best practices relevant to primary care and home-based services.
  • Ensure compliance with organizational policies, clinical standards, and state regulations.
