We are seeking a trained and dependable Home Hemodialysis Care Partner to support a female patient in her 60s receiving home hemodialysis treatments. The primary care partner (Ms. Nadia) is nearing completion of her training and is seeking a backup support professional to assist with treatments as needed. This role provides consistent, one-on-one support, not an agency rotation, allowing you to build rapport and become a trusted part of his weekly care routine. Training will be provided, including patient-specific dialysis processes, home setup guidance, and ongoing support from our clinical team to ensure you feel fully confident. This position is ideal for someone who values patient dignity, understands the unique challenges of dialysis, and wants to make a meaningful impact during a major care transition.
