About The Position

Legacy Homecare and Wellness is a trusted partner in delivering compassionate, personalized homecare so that our clients are able to live confidently and comfortably at home! In this role, you'll work under the supervision of a registered nurse, providing quality care to patients, while assisting with ADL’s and IADL’s within patient’s home in compliance with federal, state, local government regulations and company clinical policies.

Requirements

• High School Diploma or GED Required
• At least 18 years of age.
• Must be listed with no substantiated findings in the State Health Care Personnel Registry

Responsibilities

• Assist patients with ADL/IADL activities which may include personal hygiene, dressing, bathing, and other daily tasks.
• Ensures quality and safe delivery of home care services to patients/clients and families/caregivers.
• Implements plan of care formulated by Registered Nurse
• Uses client DME (durable medical equipment) and PPE (personal protective equipment) eMectively and eMectively.
• Observe, document and report activity based upon the services provided throughout the shift. Documentation may include electronic medical reporting, paper documentation, or both.
• Reporting of any emergent situations to appropriate authorities and staM in a timely manner.
