About The Position

Intensive In-Home (IIH) Team Lead – Charlotte, NC Licensed Clinician (LCSW / LCMHC / LMFT) Pathways to Life, Inc. is seeking an experienced, compassionate, and accountable clinician to lead our Intensive In-Home (IIH) program in Charlotte. If you love working directly with families in their homes, guiding a talented team, and improving outcomes without being tied to an office, this role is for you. This isn’t a crisis-driven leadership role. Our IIH program is thriving — you’ll step into a stable, well-supported team with manageable caseloads , clear workflows, and the chance to shape program quality, staff success, and family outcomes .

Requirements

  • Master’s degree in Human Services (Social Work, Counseling, Psychology, or related)
  • Fully Licensed or Associate-Level Licensed in NC (LCSW/LCSWA, LCMHC/LCMHCA, LMFT/LMFT-A)
  • Minimum 2 years of IIH experience
  • Supervisory or team lead experience preferred
  • Working knowledge of NC Medicaid IIH requirements
  • Valid NC driver’s license and reliable transportation
  • Ability to pass background check and credentialing

Responsibilities

  • Clinical Oversight Review and approve assessments, treatment plans, and progress notes
  • Ensure documentation meets NC Medicaid IIH standards and program policies
  • Monitor authorizations and continuity of care
  • Maintain fidelity to LME/MCO requirements and Pathways to Life best practices
  • Support Your Team of QPs Provide weekly supervision (individual and group)
  • Set clear expectations for productivity, documentation, and clinical quality
  • Coach early to prevent issues from becoming disciplinary
  • Support onboarding, training, and long-term professional growth
  • Program Quality & Stability Maintain compliance and clinical excellence across the program
  • Participate in audits and record reviews
  • Strengthen workflows and improve outcomes
  • Support sustainable program growth
  • Community Collaboration Serve as a clinical contact for families, schools, referral sources, and partners
  • Coordinate care and resolve escalations
  • Support referrals and intakes when needed
  • Collaborate closely with Clinical Directors, HR, and Operations
