Fairview has an immediate opening for an In Clinic Visit Facilitator to join our Primary Care team at our Fairview Lakes Clinic in Wyoming, MN. This is a 0.85 FTE (68 hours per two weeks pay period), benefits-eligible opening. Visit Facilitators are a valuable member of our care team and collaborate with providers and support staff in meeting patient care needs during and between appointments in the clinic. Visit Facilitators on our team gain experience beyond patient care by working on collaborative projects with other clinics in our service line, leading process improvement initiatives in our clinic, serving as subject matter experts in the clinic, and receive ongoing competency and skills training by our clinical leads.
