About The Position

The IMPACT Coordinator is responsible for coordinating and providing support across program operations, focusing on social service coordination and case management for homeless populations. This role involves overseeing client safety, conducting risk assessments, developing service plans, and ensuring compliance with funding requirements. The coordinator will also maintain relationships with community resources and regulatory agencies, ensuring effective outreach and reporting on program outcomes.

Requirements

  • Bachelor's Degree with at least 2 years experience working directly with homeless populations, or a high school diploma with five to ten years of social service experience.
  • Mastery of resource development and community relations.
  • Experience with diverse populations and office administration.
  • Good oral and written communication skills.
  • Strong organizational skills.
  • Functioning knowledge of Microsoft Office, including Access, Excel, and Word.

Responsibilities

  • Coordinate and provide support across program operations.
  • Oversee social service coordination and case management responsibilities.
  • Ensure ongoing security and safety of the programs.
  • Supervise the orientation of all new clients.
  • Conduct risk assessments and develop service plans.
  • Deliver treatment services to clients.
  • Identify potential homeless populations for Transitional Living and Supportive Services programs.
  • Inspect and monitor client records for compliance with contractual requirements.
  • Collect assessment and research data and process it in the database.
  • Develop and maintain relationships with funding and regulatory agencies.
  • Ensure timely completion of reports and fiscal data.
  • Maintain professional knowledge through workshops and publications.
  • Responsible for outreach plans and activities in multi-employee regions.
  • Work with program teams for accurate reporting on outcomes.
  • Act as a liaison between the program and the community.
  • Interact with clients, staff, and community members respectfully.
  • Maintain confidentiality in compliance with operational policies.
  • Attend staff meetings, supervisory conferences, and training sessions.
