About The Position
The RN Case Manager is responsible for coordinating the care of patients throughout their hospital stay, ensuring they receive optimal, timely, and efficient care. They work collaboratively with multidisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure that patients' medical, social, and emotional needs are met. The RN Case Manager also focuses on optimizing resource utilization, improving patient outcomes, and preventing unnecessary hospital readmissions.
