As an IL Clinic Nurse, you will provide professional nursing care in the Community at Rockhill Residential Living (RL) Clinic and to residents of RL in their apartments. POSITION SUMMARY: Under the direction of the Director of Residential Living, provides professional nursing care in the Community at Rockhill Residential Living (RL) Clinic and to residents of RL in their apartments.

Requirements

  • Demonstrated competency in performing nursing skills necessary to provide safe care to residents of RL (examples of needed skills are assessment of vital signs, medication monitoring and set up, routine injections, blood glucose monitoring, ostomy care and uncomplicated wound care).
  • Possesses excellent assessment and teaching skills.
  • Current LPN or RN licensure.
  • Functions within the limits of license and regulations.
  • Creativity, critical thinking, sound judgment and excellent decision-making skills as well as the ability to take initiative in planning, organizing and accomplishing tasks.
  • Ability to respond quickly, effectively, and accurately to emergencies or deadline pressures.
  • Ability to organize and prioritize activities and to function independently as well as collaboratively.
  • Commitment to the mission of the organization.
  • Ability to work in a team structure.
  • Ability to attend assigned in-service programs and other educational opportunities.
  • Ability to follow resident’s rights policies at all times.
  • Ability to follow all facility safety programs.

  • Adheres to the Values and Expectations statement and element of the compliance program.
  • Possess high moral and ethical standards.
  • Graduate of an accredited school of nursing and currently licensed as a Licensed Practical Nurse.
  • Active LPN or RN License

Nice To Haves

  • Experience in Home Health Care and Geriatrics.
  • Skilled Nursing, Hospital/acute care experience a plus

Responsibilities

  • Performs annual assessments for Residential Living residents.
  • Participates in bi-weekly RL care conferences.
  • Maintains medical records for RL residents and assists with gathering documentation for billing.
  • Performs liaison work with hospitals and discharge planners as needed.
  • Provides nursing support to RL residents regarding health needs while promoting maintenance of their independence.
  • Observes and competently assesses health status of residents a minimum of once annually.
  • Reports changes or problems to the resident’s physician as appropriate.
  • Participates in RL team meetings including resident review, transitions, and care conferences
  • Intervenes appropriately when residents experience accident or illness by referring resident for medical care or different level of care.
  • Provides coverage of the RL Clinic, including limited clinical and administrative support to visiting providers.
  • Effectively provides health teaching to clients and significant others.
  • Assists residents with appointments for lab work on site as well as with other medical appointments.
  • Documents care and needs of residents according to policy.
  • Maintains an accurate, updated medical record is maintained for each resident.
  • Promotes billable services to residents appropriately.
  • Collaborates and communicates effectively with the Director of Independent Living and other Rockhill staff to promote comprehensive and excellent care to residents.
  • Carries a cell phone while on duty in order to respond to RL emergencies.
  • Reports problems, needs and concerns appropriately.
  • Incidents are recorded promptly in Rockhill’s PEER portal.
  • Knows and adheres to the Rockhill Resident Rights policy and all elements of the Corporate Compliance policy.
  • Follows infection control policies, utilizing universal precautions effectively.
  • Completes annual in-service requirements as well as continuing education mandated by license.
  • Communicates effectively and in a caring manner with residents, co-workers, and the public.
  • Performs duties and communicates with honesty and the highest level of integrity.
  • Maintains a neat, well groomed, professional appearance within the clinic and community.
  • Demonstrates strict confidentiality of client, employee and organizational information.
  • Maintains an acceptable attendance record of 6 or less unplanned absences per year.
  • Is prompt and dependable.
  • Performs other duties necessary for the good of the organization as requested.
