About The Position

The IIH Team Leader is responsible for the admission and retention of new and existing consumers in the Intensive In-Home Program. This role provides clinical expertise and oversight for the Intensive In-Home Team and coordinates with the Director of Community Based Services. Key responsibilities include coordinating time-limited individual and intensive family treatment services to stabilize the home environment, promote reunification, or prevent out-of-home placement for youth. The team leader completes all required documentation for consumer enrollment and retention, including gathering treatment histories, developing Person-Centered Plans, maintaining authorization, and making necessary referrals. They also ensure the completion of the agency's standardized instrument panel for all admitted clients. Administratively, the position oversees program utilization, CQI, client outcomes, and marketing. The Team Leader, in conjunction with the Director of Community Based Services, is involved in recruiting, hiring, and training new staff, and provides administrative supervision to team members through weekly meetings and monthly individual supervision. The role requires 24-hour availability for crisis intervention and/or supervision.

Requirements

  • Minimum of Master’s Degree.
  • 1 year experience in a human service field.
  • North Carolina licensure or provisional licensure in one of the following disciplines: LCMHC(A), LCSW(A), LMFT(A).
  • Minimum of 1 year experience working with same or similar client population.
  • Knowledge and the ability to practically implement mental health regulations and clinical services.
  • Effective communication, decision making, analytical, interpersonal, leadership and professionalism skills and abilities.
  • Basic computer skills.
  • Basic math skills.
  • Valid North Carolina’s Driver’s License.
  • Submit to and pass pre-employment drug screening, criminal, HCPR and driving checks.

Responsibilities

  • Coordinate delivery of time-limited individual and intensive family treatment services intended to stabilize the home environment, promote reunification, or prevent the utilization of out-of-home placement (i.e. psychiatric hospital, therapeutic foster care, residential treatment facility) for the identified youth within designated time frames.
  • Complete required documentation to enroll and retain consumers in Intensive In-Home Services provided by CHA, including but not limited to: gathering previous treatment histories, development of initial/complete Person-Centered Plan, completion of all paperwork associated with maintaining authorization, and making necessary referrals.
  • Make appropriate referrals for clients who are referred and do not meet criteria to receive services provided by CHA.
  • Ensure agency standardized instrument panel is completed on all clients admitted into the program.
  • Participate in treatment or oversee the following in relation to treatment: Defuse the current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence.
  • Participate in treatment or oversee the following in relation to treatment: Ensure linkage to needed community services and resources.
  • Participate in treatment or oversee the following in relation to treatment: Provide self-help, social and basic living skills training for youth.
  • Participate in treatment or oversee the following in relation to treatment: Provide parenting skills training to help the family build skills for coping with the youth’s disorder.
  • Participate in treatment or oversee the following in relation to treatment: Monitor and manage the presenting psychiatric and/or addiction symptoms.
  • Participate in treatment or oversee the following in relation to treatment: Work with caregivers in the implementation of home-based behavioral supports.
  • Provide individual and family therapy at a frequency consistent with the client needs.
  • Work with natural and community supports (school, doctors, daycare, DSS, DJJ, faith communities, housing, food pantries, etc.).
  • Employ agency approved evidenced based practices for the delivery of services.
  • Provide administrative oversight to the program, including managing program utilization, CQI, client outcomes, and client marketing of the program.
  • Provide quality aftercare to clients after discharge.
  • Ensure program operations within a budget and spending policy.
  • Ensure program achieves monthly utilization targets and promote strategies that lead to the program exceeding budgeted production.
  • In conjunction with the Director of Community Based Services, recruit, hire, and train new staff.
  • Provide administrative supervision to the Intensive In-Home team members through weekly team meetings and monthly individual supervision.
  • Be available for crisis intervention and/or supervision 24 hours a day.
  • Maintain Licensure and actively participate in individual supervision for licensure and be responsible for keeping personal record of supervision and getting information to the state Licensing board so that Licensure will be obtained within 2 years of date of hire.
  • Complete yearly CEU's as required per licensing board.
  • Provide Clinical and Administrative oversight to the IIH team members.
  • Complete 100% of direct report’s evaluations on time (within 30 days of the effective date of evaluation) and have no old evaluations outstanding to meet expectations.
  • Supervise second level reports and have 85% of any indirect reports completed on time to meet expectations.
