About The Position

IIH Team Lead – Intensive In-Home Services Location: Charlotte, NC Lead with Purpose. Create Lasting Impact. Pathways to Life, Inc. is seeking an experienced and highly accountable IIH Team Lead to provide leadership and clinical oversight for our Intensive In-Home (IIH) program in the Charlotte, NC area. This is a key leadership role for a driven professional who is passionate about strengthening families, supporting clinicians, and delivering high-quality, person-centered care. As the IIH Team Lead, you will guide and support a team of Qualified Professionals (QPs), ensure fidelity to NC Medicaid IIH requirements , and uphold program excellence as Pathways to Life continues to expand services across North Carolina. Your leadership will directly impact service quality, staff success, and outcomes for children and families.

Requirements

  • Master’s degree in Human Services field (Social Work, Counseling, Psychology, or related)
  • Fully Licensed or Associate-Level Licensed in North Carolina (LCSW/LCSWA, LCMHC/LCMHCA, LMFT/LMFT-A, or equivalent)
  • Minimum 2 years of IIH experience (required)
  • Supervisory or lead experience strongly preferred
  • Working knowledge of NC Medicaid IIH service definitions and requirements
  • Valid NC driver’s license and reliable transportation
  • Ability to pass background check and credentialing requirement

Responsibilities

  • Provide day-to-day leadership and oversight of IIH service delivery.
  • Ensure full compliance with NC Medicaid IIH policies , LME/MCO requirements, and Pathways to Life standards.
  • Review, approve, and monitor assessments, treatment plans, progress notes, and clinical documentation.
  • Ensure timely authorizations, re-authorizations, and continuity of care for all assigned cases.
  • Supervise, coach, and support IIH Qualified Professionals (QPs).
  • Conduct individual and group supervision in accordance with Medicaid and licensure standards.
  • Address performance concerns proactively through coaching, corrective action, and professional development plans.
  • Support onboarding, training, and ongoing staff growth to promote retention and excellence.
  • Monitor documentation quality, productivity expectations, and compliance benchmarks.
  • Participate in audits, record reviews, and corrective action planning.
  • Partner with leadership to strengthen workflows, improve outcomes, and enhance program performance.
  • Serve as a primary point of contact for families, referral sources, schools, and community partners.
  • Collaborate closely with Clinical Directors, HR, and Operations leadership.
  • Support referrals, intakes, and program growth throughout the Charlotte region.
