About The Position

At Appleseed Community Mental Health Center, Inc., we are dedicated to helping families and individuals lead healthy, fulfilling lives. We offer comprehensive services in behavioral health, housing, outreach & prevention, substance use disorder, and victim support. We are currently seeking a passionate and skilled Intensive Home-Based Treatment Counselor/Social Worker (IHBT) to join our team. Role Overview: As an IHBT Counselor, you will provide vital support to youth and families directly in their homes and communities. Your role will involve managing a small caseload, offering crisis intervention, and applying trauma-informed care to help clients navigate their challenges.

Requirements

• Experience: Proven background working with youth, families, and groups on behavioral and emotional issues.
• Skills: Knowledgeable in mental illness, motivational interviewing, family systems treatment, trauma-informed care, and crisis intervention. Proficiency in DSM-5/ICD-10.
• Licensing: Bachelor’s level license required.
• Transportation: Must have a car, maintain required insurance, and possess a clean driving record.
• Commitment: Strong dedication to client rights and personalized care.

Responsibilities

• Deliver home-based therapy and support to a caseload of 4-6 youth/families.
• Provide flexible, on-call crisis response and face-to-face support as needed.
• Assess and address safety concerns, risk behaviors, and develop personalized safety plans.
• Apply comprehensive trauma-informed mental health treatments and best practices.
• Engage in individual and group supervision, complete core training, and participate in ongoing professional development.
• Collect feedback to ensure high-quality service and contribute to a supportive team environment.
