POSITION SUMMARY: Reports to the Director of Home Services (DHS). The IDT Team 2 Manager is responsible for the effective and efficient daily supervision of their clinical team including oversight of all patient care services, continuity and quality of care for both facility and home patients, performance of community outreach and service promotion to support business growth and community impact. The IDT Team Manager has the responsibility to motivate and inspire staff and foster a positive work environment.

  • Knowledge of private insurance, Medicare and Medi-Cal regulations.
  • Knowledge of State and Federal regulations and accreditation standards.
  • Must be able to work well under pressure, meet multiple priorities and competing deadlines.
  • Demonstrates cooperative behavior with colleagues and management staff.
  • Current California registered nurse license.
  • At least 2 years’ experience working as registered nurse caring for the terminally ill.
  • MSN/BSN preferred; AD (associate degree) with 2-3 years of direct supervisory experience in hospice is acceptable alternate qualification.
  • CPR certification required.
  • Current California driver’s license and reliable vehicle required

Nice To Haves

  • Certified Hospice & Palliative Care clinician preferred.
  • Experience working as an RN in a Skilled Nursing Facility preferred.

Responsibilities

  • Evaluate the staffing requirements and recommends changes in the staffing patterns to DHS.
  • Prepare the monthly schedule in coordination with the scheduler and other Team Managers as appropriate.
  • Review productivity report, position control and SHP data on a regular basis to ensure agency goals are being met.
  • Uphold and enforces company policies and brings questions and recommendations for change to senior leadership via DHS.
  • Review and approves timecards, and pays particular attention to overtime, and missed meals. Utilizes the EMR and HRIS systems to regularly track and address inconsistencies in a timely manner.
  • Review and approves time off requests and ensures adequate patient care coverage in coordination with the scheduler and other team managers as appropriate.
  • Review and ensures that assignments and scheduling of all team members effectively meet the needs of their assigned patients, while remaining compliant with organization policies and procedures.
  • Recruit, screen, and interview candidates for open positions and participates in orientation of new personnel in partnership with the Education Department. The IDT manger will also be responsible for signing off new employees to ensure they are ready to function independently.
  • Coordinate with the intake/community liaison nurse in the assignment of case managers available for new admissions. Participates in the establishment, revision and implementation of administrative policies and procedures.
  • Participate in collective bargaining as required and works with the Chief Human Resources Officer to address grievances.
  • Ensure accountability of direct reports through regular tracking and documented coaching, training, and counseling.
  • Facilitate or delegate responsibility for interdisciplinary team meetings (IDT) that effectively meet regulatory and organization standards.
  • Prepare the agenda and conducts monthly team meetings.
  • Coordinate home health aide oversight by case managers.
  • Understand the contracts with providers for delivery of services, such as pharmacy, durable medical equipment, and medical supplies, and monitors pharmacy and DME expenses. All outliers identified will be brought to the attention of the DHS along with an action plan for a solution to remedy the situation.
  • Ensure compliance with Federal and State regulations and accreditation standards.
  • Monitor documentation and ensures compliance with state and federal regulations, accreditation standards, corrective action plans and agency policies and procedures.
  • Participate in education support relevant to EMR revision and monitors staff for compliance with any changes.
  • Participate in the development of the Clinical Services annual goals and objectives, including quarterly evaluations, offer support, identify strategies and monitor staff performance toward goals and objectives.
  • Participate in the ongoing evaluation of the clinical component of the computerized patient care system and brings attention to perceived barriers interfering with efficient and timely documentation.
  • Ensure staff members document in the home/facility and if unable, ensures that documentation is completed per policy.
  • Perform service recovery and completes complaint investigations within organization’s stated timelines.
  • Actively participate in Performance Improvement Projects.
  • Collaborate with Quality &Education to support the QAPI program by reporting out on audits relevant to the home program.
  • Assist the DHS with budgeting and financial planning.
  • Participate in Administrator On-Call (AOC) rotation.
  • Perform field observations approximately 3 times per year of each team member.
  • Provide clinical coaching to staff through joint field visits.
  • Identify educational needs of staff and establishes opportunities to address needs, including referrals to the Education team.
  • Coach and develop staff to assist them in meeting their full potential.
  • Hold staff accountable to performance markers up to and including disciplinary action.
  • Act as resource to the staff regarding clinical issues that arise.
  • Participate in the quality review process regarding utilization management, including review of new patient documentation within 3 days following admission & recertification review of documentation for ongoing patients.
  • Ensure the quality of patient care and patient safety practices.
  • May provide direct care of patients as required during times of critical staffing shortage, including performing Admissions, as needed.
