The IDD Service Coordinator is responsible for monitoring and coordinating care and services for individuals with intellectual and developmental disabilities (IDD). This role involves assessing needs, developing service plans, facilitating service linkages, and providing crisis intervention and prevention supports. The coordinator advocates for clients to ensure they achieve their personal goals and maintain a high quality of life, working independently under limited supervision.

  • Monitor care and services provided to IDD individuals to ensure quality of life.
  • Conduct assessments to identify personal outcomes and service needs.
  • Develop and facilitate service plans and linkages for clients.
  • Provide crisis intervention and prevention supports as needed.
  • Document progress and satisfaction with services in a timely manner.
  • Meet with clients and their representatives to monitor outcomes and services.
  • Ensure compliance with local, state, and federal regulations.
  • Advocate for clients' rights and promote individual choice.
  • Maintain confidentiality of sensitive records and client information.
  • Participate in community activities and meetings as needed.
  • Bachelor's degree in psychology, social work, nursing, or related field.
  • At least 1 year of experience with individuals with IDD or mental illness preferred.
  • Valid Texas Driver's license and auto liability insurance required.
  • Ability to pass criminal history and background checks.
  • Must obtain QIDP certification within 6 months of hire.
  • Experience in case management or service coordination.
  • Knowledge of trauma-informed care principles.
  • Bilingual skills for translation purposes.
