We are hiring for the IDD Service Coordinator position at RHA Health Services. This role is designed for a positive and personable individual looking to make a real difference in the lives of people with intellectual, developmental disabilities, and those facing mental health and substance use challenges. The Clinical Case Manager will assist in the management, development, and oversight of the individual support plan (ISP), ensuring its implementation and determining progress. The position involves collaboration with various team members including residential team leaders, support coordinators, clinical staff, and nurses.

  • Assist with coordinating IDT meetings and the ISPs
  • Supervise and monitor the implementation of the Individual Support Plan (ISP)
  • Ensure a streamlined admission process
  • Respond to incident reports through collaboration with staff to develop root cause analysis and response plans
  • Analyze and summarize progress status by determining program appropriateness and recommending changes
  • Communicate regularly with the Support Coordinator and other members of the circle of support if amendments to the ISP are needed
  • Document all communication
  • Regularly spend time at the location where services are being performed
  • Conduct regularly announced and unannounced visits in residential homes to ensure policy and procedures are being implemented appropriately
  • Provide coverage if there is a shortage of direct support personnel while continuing to manage duties
  • Assist in on-call rotation as specified by the Administrator, including weekends and holidays
  • Train and conduct in-services for direct care staff as needed
  • Conduct internal peer reviews, observations, and other internal QA assessments
  • Conduct CQL's Personal Outcome Measures
  • Support key components of the person's health record for accuracy and timeliness
  • Perform all other duties as assigned
  • Bachelor's degree in Social Work, Social Services, Human Services or Special Education required; Master's degree a plus
  • Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master's degree
  • Working knowledge of applicable regulations related to service offerings
  • Valid driver's license, auto insurance, and reliable transportation
  • Supervisory/management experience and experience writing person-centered (individualized service) plans preferred
  • Must be at least 18 years of age
