The IDD Service Coordinator position at Bluebonnet Trails Community Services is designed to provide essential services to children and adults with developmental disabilities.   • Provide services to children and adults with developmental disabilities based on the Person-Centered Thinking model.
  • Conduct comprehensive assessments of individual needs and desires.
  • Develop and implement service plans tailored to individual requirements.
  • Facilitate service linkages and monitor their effectiveness.
  • Provide crisis intervention and prevention supports as needed.
  • Advocate for individuals in job readiness and employment activities through the Employment First project.
  • Participate in interagency planning and CRCG meetings to bridge service gaps.
  • Document services provided and maintain accurate records of interactions with individuals and families.
  • An associate degree or higher in social, behavioral, or human service field, or a high school diploma with two years of relevant experience.
  • Experience working with individuals with intellectual or developmental disabilities is preferred.
  • Ability to work independently and exercise professional judgment in various situations.
  • Strong communication skills to interact effectively with individuals, families, and service providers.
  • At least one year of experience working with individuals with intellectual/developmental disabilities.
  • Bachelor's or Master's degree in a relevant field.
  • Bilingual skills (English/Spanish/ASL) are a plus.
  • Knowledge of community resources and Medicaid programs.
