Supervises, integrates and manages individual care plans, records progress and initiates periodic reviews. Investigates modifies and adjusts care plans. Collaborates with medical and other members of the clinical team by performing the following duties. Relies on knowledge, experience, and judgment with limited direction and supervision. May supervise the work of others. Typically reports to the Administrator. Education, Licensure, and Experience required for the position include: Qualified Professional must have a four-year degree in a human service field and two years of experience or a four-year degree in other fields with four years of experience or hold a license in a human service profession. A graduate of a college or university with a Master’s degree in a Human Service field preferred.
