About The Position

The IDD Continuity of Care Coordinator at Bluebonnet Trails Community Services is responsible for providing continuity of care and intake/access coordination for individuals with intellectual and developmental disabilities (IDD) across multiple counties. This role focuses on supporting individuals transitioning to or from institutional care, ensuring their ongoing service needs are met, and empowering them to achieve their life goals. The coordinator will engage with families, agencies, and interagency personnel to facilitate service linkages and monitor the effectiveness of service plans.

Requirements

  • An associate degree or higher in social, behavioral, or human service field, or a high school diploma with two years of relevant experience.
  • Experience working with individuals diagnosed with Intellectual or Developmental Disabilities.
  • Good clinical judgment and ability to work effectively in a fast-paced setting.
  • Strong computer and documentation skills.

Nice To Haves

  • At least one year of experience working with individuals with intellectual/developmental disabilities.
  • Knowledge of community resources and Medicaid Waiver programs.
  • Bachelor's Degree or Master's Degree in a relevant field.
  • Bilingual (English/Spanish/ASL).

Responsibilities

  • Provide continuity of care and intake/access coordination for individuals with IDD.
  • Coordinate ICF placement and authorization, including Permanency Planning for individuals under age 22.
  • Support discharge planning for individuals in state hospitals or SSLCs.
  • Develop transition plans to ensure ongoing service needs are met.
  • Participate in interagency planning and CRCG meetings to bridge service gaps.
  • Conduct intake services for children and adults with Autism and IDD.
  • Complete eligibility screenings for various funding streams and referrals to services.
  • Gather supporting documents and complete developmental history for scheduling Determination of Intellectual Disability (DID).
  • Provide crisis intervention/prevention supports and advocacy services.
  • Travel within the service area to meet with individuals and families.
