About The Position

Easterseals, a national nonprofit, has supported individuals with disabilities for over 50 years. Its services for intellectual/developmental disabilities and mental health conditions provides daily living support and community-based programs that teaches individuals with special needs valuable life skills through instruction and real world experiences.

Requirements

• Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services or a related field from an accredited college or university.
• At least 2 years of experience with community-based mental health services is preferred.
• Strong computer skills (Microsoft Word & Excel).
• Valid driver’s license and acceptable driving record.
• Excellent communication, planning, and organizational skills.
• A compassionate, flexible, and client-focused approach to care.

Responsibilities

• Through community outreach within Somerset County, NJ, provide ongoing support to consumers to help them function independently and access mental health services.
• Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
• Assist consumers in accessing needed services and ensure transportation to appointments.
• Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
• Assess the need for crisis intervention and assist in psychiatric emergency situations.
• Encourage wellness and recovery while maintaining individual dignity and respect.
• Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.
