Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking an ICM Licensed Medical Social Worker. The CM Licensed Medical Social Worker provides social services to members with challenging medical and behavioral health issues. This role involves receiving referrals for members who screen positive for depression and are in need of additional behavioral health support, as well as supporting post-discharge follow up for referred members discharged from the hospital with active behavioral health needs. The position fosters interaction between members and medical or behavioral health providers and serves as a liaison to the behavioral health ASO to coordinate care. The social worker collaborates with the behavioral health ASO to coordinate monthly co-management meetings and assure optimal clinical outcomes for co-managed members. Additionally, the social worker maintains a caseload, monitoring patient progress and facilitating appropriate medical and behavioral health follow-up as necessary.

  • Provides education and support to Care Management staff on handling behavioral concerns and crisis situations.
  • Participates in care coordination and community collaborative meetings as needed.
  • Master’s degree in Social Work or related field.
  • 3 to 5 years of related experience.
  • Direct experience in handling caseload with chronic medical and behavioral health issues.
  • Familiarity with health care services delivery to economically disadvantaged populations.
  • Working knowledge of community resources and community service needs of individuals and families in lower socioeconomic levels.
  • Broad understanding of cultural competency and HIPAA requirements.
  • Excellent oral and written communication skills.
  • CT Clinical Social Worker’s license must be current and unrestricted.
  • Valid and unrestricted state motor vehicle operator's license and reliable transportation.
