About The Position

The Human Trafficking Case Manager serves as a frontline responder and long-term advocate for survivors of human trafficking across Lake County. This position operates within the Lake County Task Force and collaborates directly with law enforcement, prosecutors, and community partners to ensure survivors receive immediate crisis support and comprehensive, trauma-informed services. This is not a desk job. This is field-based, relationship-driven work that requires emotional intelligence, crisis readiness, and strong system navigation skills. You will respond to law enforcement calls, conduct safety and danger assessments, develop individualized service plans, and walk alongside survivors as they rebuild stability, independence, and safety. This role reports to the Director of Human Trafficking and works collaboratively across agency programs.

Requirements

  • Bachelor’s degree in Social Work, Counseling, Education, or a closely related human services field
  • Experience in crisis intervention, case management, or victim advocacy
  • Ability to work flexible hours, including evenings, weekends, and on-call rotation
  • Strong written documentation and communication skills
  • Valid driver’s license and reliable transportation

Nice To Haves

  • Bilingual (Spanish/English strongly preferred)
  • Experience working with survivors of trafficking, domestic violence, or trauma
  • Familiarity with public benefits systems and victim compensation processes

Responsibilities

  • Participate in 24-hour on-call rotation, including one weekend every 6–8 weeks (with two weekdays off)
  • Respond to referrals from Homeland Security, FBI, Lake County Sheriff, and Lake County State’s Attorney’s Office
  • Provide immediate onsite or offsite crisis intervention
  • Conduct danger assessments and safety planning at intake and throughout services
  • Conduct trauma-informed intakes and comprehensive needs assessments
  • Develop and monitor individualized service and safety plans
  • Provide ongoing advocacy, documentation, and follow-up across systems
  • Assist with crime victim compensation claims and related documentation
  • Ensure timely, accurate documentation in accordance with agency and funding requirements
  • Provide intake and supportive services in areas including: Shelter and housing placement Public benefits enrollment (TANF, SNAP, Medicaid, childcare assistance) Employment readiness and workforce preparation Education access and documentation assistance Family support and reunification Legal advocacy and court accompaniment Medical and mental health coordination Substance use treatment referrals Transportation and basic needs support
  • Attend Lake County Human Trafficking Task Force meetings, Steering Committee, Coalition meetings, and Victim Identification Subcommittee
  • Foster strong partnerships with law enforcement, legal partners, healthcare providers, and community agencies
  • Participate in staff meetings, case consultations, and required trainings
  • Maintain confidentiality consistent with agency and survivor safety standards
