About The Position
This position within the Extended Foster Care (EFC) team, reports to the EFC Review Supervisor and is responsible for coordination and facilitation of the administrative review panel for young adults in extended foster care. The position will be responsible for ensuring that young adults in the extended foster care program have the supports, services, and the quality case management needed to successfully transition to adulthood. They will provide technical assistance to support best practice in the field, and to promote program development within the agency. This job will be responsible for modeling the Youth Thrive culture, and philosophy when working with young adults and/or DCS Specialists/EFC Coaches. They will be conducting case reviews, to include outreach to the young adult, DCS Specialist, EFC Coach, community supports and serve as an advocate for the young adults and/or DCS Specialists/EFC Coach.
