About The Position
Dementia Care Coordinator will provide coordination of care to individuals with dementia, their families and caregivers. Performs professional human service work that contributes to the efficient and effective operation of dementia care services across the Office on Aging and Independence. The care coordination will include, but not be limited to, options counseling, education on dementia, dementia screenings, information and referrals to community-based organizations, care transitions coaching, organization of support groups, behavioral symptom management training and expert consultation and eligibility assistance. This is a part-time, benefited position working an average of not more than 30 hours per week. This position will serve at the pleasure of the Appointing Authority.
