About the position

The primary purpose of this position is to serve as a liaison between families, legal representatives, and/or significant others of residents served by O'Berry Neuro-Medical Treatment Center. The role involves developing one-to-one relationships with residents and their families through visits, activities, correspondence, and phone calls. Responsibilities include counseling families related to residents' physical limitations and issues associated with death and dying, notifying families of any emergency issues related to residents, assisting in new applicant screening and the admission process, and participating as part of the interdisciplinary team in resident assessment and care plan processes. The position also requires writing quarterly and annual progress notes for each resident.

Responsibilities

  • Serve as a liaison between families, legal representatives, and significant others of residents.
  • Develop one-to-one relationships with residents and their families through visits, activities, correspondence, and phone calls.
  • Counsel families related to residents' physical limitations and issues associated with death and dying.
  • Notify families of any emergency issues related to residents.
  • Assist in new applicant screening and admission process.
  • Assist the Transition Coordinator with admissions as needed.
  • Participate as part of the interdisciplinary team in resident assessment and care plan processes.
  • Write quarterly and annual progress notes for each resident.

Requirements

  • Bachelor's degree in a human services or related field from an appropriately accredited institution.
  • One year of experience providing and/or developing person-centered care plans for individuals with intellectual disabilities/co-occurring disorders.
  • Thorough knowledge of the concepts, practices, and techniques used in the field of neurocognitive disorders.
  • Knowledge of Certified Medicaid Services (CMS) or OBRA regulations and methods of ensuring compliance.
  • Demonstrated ability to analyze data and determine trends and patterns.
  • Demonstrated ability to write person-centered habilitation plans or care plans and reports.
  • Demonstrated ability to develop cooperative working relationships with a variety of people.
  • Demonstrated ability to work as part of an interdisciplinary team.

Nice-to-haves

  • Advanced degrees in human services may be substituted to meet educational requirements.
