About The Position

The Human Services Program Consultant II (HSPC II) is responsible for providing specialized case management services to pregnant women, parents of at-risk infants, and women between pregnancies. This role focuses on coordinating service delivery, conducting assessments of clients' medical and social needs, and ensuring continued involvement in prenatal and child health care. The consultant will engage with families to promote health, well-being, and self-sufficiency through various support services and community resources.

Requirements

  • A bachelor's degree from an accredited college or university in social work, social sciences, or a health-related field.
  • A minimum of three years of verifiable professional experience in providing social work, health education, nursing, or counseling services.
  • At least one year of verifiable professional experience conducting interviews or assessments in a Health or Social Services setting.
  • Current Florida Department of Health employee status.
  • Valid Florida driver's license or state identification card.

Nice To Haves

  • Knowledge of counseling techniques and practices.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Basic computer skills, including keyboarding, WINDOWS, and MS Suite.

Responsibilities

  • Provide specialized case management services to pregnant women and parents of at-risk infants.
  • Conduct assessments of clients' total medical and social needs.
  • Coordinate service delivery and care for participants in the Bellies, Babies and Beyond program.
  • Engage participants to promote continued involvement in prenatal and child health care.
  • Facilitate planning and problem-solving with families to address their needs and priorities.
  • Provide information, education, and encouragement to motivate families to change at-risk situations.
  • Develop and monitor plans of care to ensure concerns are addressed and promote healthy outcomes.
  • Collaborate with other providers to ensure continuity and coordination of care.
  • Document client services and maintain accurate electronic records.
  • Participate in staff development and training programs.
