About The Position

HUMAN SERVICES COUNSELOR III Your Specific Responsibilities: · Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact. · Provide Case Management to pregnant women and infants referred to the Healthy Start Program. · Provide face to face contact with clients either in the OB provider’s office or in the client’s residence. · Provide Parenting Education one on one with clients. · Provide Breastfeeding Education and Tobacco Cessation Education to individual clients. · Document all client contact information in the web-based computer application known as the Well Family System · Provide referrals to community resources to supplement Healthy Start services.

Requirements

  • Fluency in English and Spanish
  • Experience working with pregnant women and children.
  • Problem solving skills related to advancing client self-sufficiency.
  • Ability to work well with others as a team.
  • Computer skills: competency and experience.
  • Experience in interviewing and assessing to determine the needs of the client.
  • Knowledge of community resources.
  • Valid Florida Driver's License
  • Minimum of four-year college degree in one of the following areas; Social Sciences; a health-related field such as Nursing, Health Education, Health Planning, or Health Care Administration; or Social Work as required by FL Healthy Start Standards and Guidelines (Chapter 6-2)
  • Bilingual with fluency in English and Spanish
  • At least 1 year experience working with high-risk pregnant women and/or families
  • Must have reliable and dependable transportation every day for home visitation purposes

Nice To Haves

  • 1 to 3 years of experience working with families
  • 1 to 3 years home visiting experience
  • 1 to 3 years case management experience
