As a frontline defender against abuse, this position plays an essential role in safeguarding the dignity and well-being of vulnerable seniors and individuals with disabilities, ensuring that every voice is heard, and every allegation is thoroughly investigated. The purpose of this position is to determine initial and on-going Medicaid eligibility for long term services and supports (LTSS); shall include determining initial and on-going financial eligibility for Oregon Project Independence-Medicaid (OPI-M) and Family Caregiver Assistance Program (FCAP). Additionally, the purpose of this position is to develop and implement service plans, to coordinate the provision of services for community-based care and nursing homes, and to regularly review and update Medicaid eligibility and monitor service plans.
