Closing Date: February 18th, 2026, or upon receipt of the first 50 applicant submissions Human Service Worker - Children’s Long-Term Support Waiver Children Youth and Family Services– Coggs location Hourly Range: $25.37 - $38.03 The Children’s Long-Term Support (CLTS) waiver program is a Medicaid Home and Community Based Waiver program which provides services for children with disabilities. The waiver program allows Wisconsin to use Medicaid funding for community-based supports and services to promote inclusion, access, and empowerment. The program is administered by Milwaukee County Department of Health and Human Services, Children, Youth and Family Services for residents of Milwaukee County. Job Summary: Under the direction of the Supervisor, the purpose of the Human Service Worker CLTS is to facilitate the Federal Home and Community Waiver services for families. This position will assess a child's ongoing disability related needs and develop a service plan in collaboration with the child(ren) and family. CLTS HSWs oversee services, develop service authorizations, meet monthly with families as well as determine a child's eligibility for services each year. CLTS HSWs follow Medicaid Regulations, advocate for families, and ensure equitable service delivery.

Requirements

  • Resident of the state of Wisconsin
  • Valid Wisconsin driver’s license with access to an insured and reliable vehicle; both maintained throughout employment
  • Associate degree or higher from an accredited college or university in the field of Social Work, Psychology, Human Services, or in a closely related field
  • At least one (1) year of experience working with Children with Disabilities, case management, or related experience.
  • Advanced knowledge of all related computer and software applications, such as word processing and spreadsheets.

Nice To Haves

  • Licensing or formal training in Social Work preferred.
  • Long-Term Care Functional Screen Certified preferred
  • Preference for candidate who has Certificate or Completion of CLTS Waiver Basics

Responsibilities

  • Assess the functional and program needs of children to identify their disability related needs. Review disability documentation to assess a child's daily living activity and independence to identify areas of potential development.
  • Utilize skills to complete eligibility assessment as required by state waiver manual requirements. Review diagnostic and evaluation reports to complete re-certification and monthly reviews and reassess program eligibility on an annual basis.
  • Work collaboratively with parents and providers to develop individual services plans based on the child's disability related needs. Collaborate with school districts, doctors, as well as community providers to respond to the needs of the children and their families.
  • Complete monthly home visits to ensure the child's disability related needs are met. Review and develop safety and service plans to meet the child's unique needs. Cultivate resources for families. Complete case notes along with documentation of critical incidents and hospital visits.
  • Document all contacts including progress notes to demonstrate ongoing service provision. Document Third Party claims activities, service delivery notes for each service, progress, and planning, as well as resources and referrals as appropriate. Ensure Medicaid rules are followed.
  • Participate in child case staffing and other meetings to identify informal and formal support to coordinate with federal and state funding requirements. Work collaboratively with county and community services to coordinate services that are goal and outcome based.
  • Identify appropriate referrals and resources for children and families based on the child's disability related needs. Advocate and support children and their families.
  • Perform other duties as assigned.
