The HSS Field Service Coordinator, RN - IDD role will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Our team is made up of Behavioral Health Specialists, RNs, and Social Workers dedicated to helping members achieve their health care goals. Position is field based with home-based office. If you are located in or within commutable driving distance to Spring Valley Village or Bellaire, TX area, you will have the flexibility to work remotely as you take on some tough challenges.

Requirements

  • Active, unrestricted RN license in the state of Texas
  • 2+ years of experience working with people with Intellectual and Developmental Disability (IDD)
  • 2+ years of experience working within the community health setting or in a healthcare role
  • Intermediate level of proficiency working with MS Word, Excel and Microsoft Outlook
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
  • Access to reliable transportation, valid US driver’s license, and the ability to travel up to 75%25 of the time in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
  • Reside in or within commutable driving distance to Spring Valley Village or Bellaire, TX, and surrounding areas

Nice To Haves

  • 1+ years of experience with long term care services and support, Medicaid, or Medicare
  • Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
  • Bilingual in English and Spanish
  • Proven ability to create, edit, save and send documents, spreadsheets and emails

Responsibilities

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, at least restrictive level of care
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team

  • Make outbound calls and receive inbound calls to assess members' current health status, identify gaps or barriers in treatment plans
