About The Position

The HSS Clinical Coordinator at UnitedHealth Group is responsible for managing a panel of members with complex behavioral health needs. This role focuses on care management and coordination activities that address the medical, behavioral, and socioeconomic needs of members, promoting appropriate service utilization and improved quality of care. The coordinator will work closely with the provider community to reduce fragmentation in care and will provide clinical guidance within the team. The position emphasizes health equity and aims to address health disparities among members.

Requirements

  • Must possess an MSW or a Master's degree in a behavioral health-related field with a current, unrestricted license, or a BA in a behavioral health field with 5+ years of direct clinician experience, or an LPN with 5+ years of experience serving individuals with behavioral health conditions.
  • 2+ years of experience serving individuals with behavioral health conditions (mental health/substance use disorders).
  • 2+ years of experience serving Medicaid populations.
  • Familiarity with state-specific behavioral health Medicaid benefits and local support organizations.
  • Ability to travel 15% of the time for the role.
  • Access to a designated quiet workspace at home with secure internet connection.

Nice To Haves

  • Experience serving individuals with co-occurring disorders.
  • Experience working in team-based care.
  • Training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, or Person-Centered Care.
  • Bilingual in Spanish or other relevant languages.
  • Experience in Managed Care.
  • Lived experience in managing chronic health or behavioral conditions.

Responsibilities

  • Serve as primary care manager for members with high behavioral needs.
  • Engage members through various modalities to complete comprehensive needs assessments.
  • Develop and implement individualized, person-centered care plans.
  • Collaborate with internal care teams, providers, and community resources to meet care plan needs.
  • Monitor and update care plans based on member feedback and progress.
  • Assist members in developing personal wellness and health crisis plans.
  • Provide psycho-education regarding conditions, medications, and healthcare system utilization.
  • Offer coaching and supportive counseling for self-management and lifestyle changes.
  • Facilitate referrals and linkages to appropriate services and providers.
  • Perform activities to address HEDIS/STAR gap closure.
  • Support proactive discharge planning after hospital admissions.
  • Educate members on Medicaid benefits and coordinate care management activities.
  • Document all care management activities in the clinical care management system.
  • Provide guidance to team members on behavioral health conditions and best practices.
