In this RN Nurse Care Coordinator role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Our team is made up of RNs, Social Workers, Behavioral Health Specialists and others dedicated to helping members achieve their health care goals. If you are located in or within commutable driving distance to Cypress, Katy, Jersey Village, or Memorial City, TX areas, you will have the flexibility to work remotely as you take on some tough challenges. Based working from home and must be able to travel about 75%25 of the time in NW Houston, Texas and surrounding area.
