This entry-level, position is designed for individuals interested in community health, social determinants of health, and housing support services. You will perform routine, structured tasks under direct supervision, assist with documentation and referral workflows, support prior authorization processes, and interact telephonically with a small caseload of members. This role offers ongoing learning, shadowing, and hands-on development toward CHW or Housing Navigator career pathways. Requirements

  • High School Diploma/GED
  • 1+ years of experience in customer service, community programs, or administrative support
  • Intermediate level of proficiency with Microsoft Office (Word, Excel, Outlook)
  • Access to reliable transportation and & valid US driver's license

Nice To Haves

  • Interest in pursuing Community Health Worker (CHW) certification
  • Experience in healthcare, social services, housing programs, or SDOH-related work
  • Experience making routine telephonic outreach to members
  • Knowledge of local community, housing, or support service resources
  • Familiarity with documentation-heavy tasks, data entry, and electronic record systems
  • Strong attention to detail and ability to follow established procedures and workflows
  • Bilingual (English/Spanish)

Responsibilities

  • Provides telephonic support to members with housing or social needs under direct supervision
  • Manages a reduced, limited caseload (20-30 members) focused on routine follow-up tasks
  • Reviews incoming referrals and assigns them to appropriate team members following established workflows
  • Ensures member documentation is accurate, complete, and submitted according to program and state requirements
  • Supports preparation, submission, and tracking of prior authorizations for applicable programs
  • Performs routine data entry, document uploading, and record maintenance across internal systems
  • Assists with generating reports, tracking metrics, and maintaining internal logs or spreadsheets
  • Conducts outreach to gather missing information or documents from members or providers
  • Maintains program compliance by following established procedures and escalating non-standard issues
  • Supports day‑to-day administrative operations, including scheduling, inbox monitoring, and routing inquiries
  • Participates in training, shadowing, and skill‑building activities to develop CHW and Housing Navigation competencies
  • Collaborates with team members to support overall housing, justice, and community health program operations
  • Demonstrates reliability, clear communication, attention to detail, and willingness to learn within a supervised role
