About The Position

PathPoint is seeking a full-time Housing Specialist to join our Behavioral Health division in Santa Maria, CA . The position is community based/remote and will cover the following locations: Santa Maria, Guadalupe, Lompoc, Buellton, and subject to change to surrounding Santa Barbara County Cities. In this role, you will provide direct support to individuals experiencing or at risk of homelessness by guiding them through the process of securing and maintaining safe, stable housing. This role is part of a client-centered program that integrates Housing Transition Navigation Services (HTNS) and Housing Tenancy and Sustaining Services (HTSS). The specialist works closely with members and community partners to promote long term housing stability, improve health outcomes, and enhance overall well-being through comprehensive care coordination. Additionally, ensure our services are holistic, client-centered, and trauma-informed. Schedule: Monday-Friday, 40 hours per week with your choice of five 8-hour shifts or four 10-hour shifts. Daily schedule will be based on program needs and business hours of contacts and vendors. Compensation: This is an hourly, non-exempt position with a starting wage of $24.00 per hour and the position's pay range is $24.00 to $29.00 per hour. The pay range is subject to change based on a variety of factors.

Requirements

  • Bachelor's degree in related field such as sociology, psychology, or social work and the following work-related experience preferred or a combination of a bachelor's degree or HS Diploma or equivalent and 2 years of the following work-related experience.
  • Experience in a social service setting working with families and/or individuals with mental health diagnosis, developmental disabilities, substance use disorder, homelessness or at risk.
  • Knowledge of housing subsidy programs, resources, Coordinated Entry System (CES), Housing Authority, and tenant rights.
  • Knowledge of case management planning and systems, care plans, assessments, and the ability to record data on excel spreadsheets.
  • Good written and oral communication.
  • Must have a valid California driver's license with satisfactory driving record.
  • Culturally aware, acknowledges diverse employee backgrounds, possesses skills to collaborate across cultures, and avoids biased assumptions based on cultural differences.

Responsibilities

  • Facilitate the connection and enrollment of clients into eligible and applicable programs and resources.
  • Assist members in locating and securing housing, including completing rental applications, gathering documentation, and coordinating move-in logistics.
  • Coordinate services across housing, medical, behavioral health, and social support systems to meet member needs holistically.
  • Facilitate communication among members, families, service providers, and health plans to ensure continuity of care.
  • Maintains an up-to-date list of housing opportunities and resources.
  • Conduct regular client meetings and document progress.
  • Provide home and field-based services as needed.
  • Prepare reports and documentation as needed for planning and/or billing and reporting requirements.
